Discipline ii Three platforms, compared

Non-surgical skin tightening.

A side-by-side comparison of the three FDA-cleared platforms that define the category. Each operates at a different tissue depth and addresses a different clinical profile.

§ 01
The category, defined

Skin tightening platforms address tissue laxity by delivering controlled thermal energy to the dermis or subdermis, triggering an injury response that stimulates new collagen production over the following months. The platforms differ in three important ways: the form of energy used (radiofrequency or focused ultrasound), the depth at which the energy is delivered, and whether that depth is fixed or adjustable.

The wrong platform on the right candidate produces no result. The right platform on the wrong candidate produces no result, or worse. Selecting the appropriate platform requires an in-person evaluation that establishes tissue thickness, laxity grade, and the specific layer that needs to be addressed.

The comparison
i.
Thermage® FLX
Monopolar RF
ii.
Ultherapy®
Microfocused ultrasound
iii.
Sofwave®
Synchronous parallel ultrasound
Mechanism
Bulk monopolar radiofrequency heats deep dermis and subdermis volumetrically.
Focused ultrasound creates discrete coagulation zones at fixed depths.
Parallel ultrasound beams heat mid-dermis at a uniform 1.5 mm depth.
Treatment depth
Up to 4.3 mm, adjustable
1.5, 3.0, and 4.5 mm
1.5 mm, fixed
Best suited for
Larger surface areas, body laxity, abdomen, arms, thighs, jowls.
Lifting the face, brow, and neck through deeper SMAS-layer tightening.
Fine lines and mid-dermal laxity in the face and neck.
Visualization
No real-time imaging; bulk heating throughout treated zone.
Real-time ultrasound imaging shows tissue layers being treated.
No imaging required at fixed depth.
Sessions
Single, repeat in 12 to 18 months
Single, repeat in 12 to 18 months
One to two, repeat annually
Final result visible

Editorial note

A 50-year-old with significant submental laxity is a different candidate than a 35-year-old with early jawline softening. The first may benefit from Ultherapy's deeper SMAS-layer reach; the second may be better served by Sofwave's mid-dermal precision. Recommending one platform without first establishing the candidate is the signature of a single-machine practice.

§ 03
Where this is practiced

Perfect Body Laser & Aesthetics® is a 16-time award recipient from Solta Medical, manufacturer of Thermage®, recognized for the most accumulated experience in skin tightening and fat reduction in the United States. The center operates Thermage® FLX, Ultherapy®, and additional FDA-approved tightening platforms, allowing the team to match the technology to the candidate rather than the other way around.
